Looking For Quotes On The Following Site Design And Installation

I am looking for a designer/programmer to complete a project that I am looking to begin this month. It will be a very basic site that I will be using for PPC marketing. As I said, it will be a simple site, nothing extravagant and cost will play a factor in who I decide to go with. Ok now for details on what I am looking for.

At the top I will be putting a header (will have this designed by this week) which will measure 900x225. I would like the menu on the left side and would like the following menu buttons:

Home page
Trials
- Business
- Health & Beauty
- Entertainment
Mortgage
Education
Finance
Sweepstakes
Dating

The 3 options under Trials I would like as sub-options, where you click on Trials and they open up. Trials itself will not go to a specific page, just the three options underneath it.

Under the menu, I will be putting banner ads, same width as the menu.

The home page will be a brief description of what the site entails, nothing fancy.

For the individual pages themselves I will be including a banner ad with a brief paragraph explaining the offer. I would like a simple control panel set up where I can just input the banner url and link associated with it as well as the text, and it will show up on the page properly instead of my having to format it myself. The banners will be standard 468x60 banners. I would prefer the banner be at the top of the individual offer with the text underneath it. Each offer should be separated, kind of like separate boxes. Tough to explain but actually pretty simple.
