Do We Have Any Php/mysql Programmers Here?

I have been working hours on end trying to get UltimateXchange up and running and have run into a snag in the exchange payment verification script so can you tell me what I am doing wrong here? I appreciate any help.

With the above I am just trying to pull some info from the database so I can enter it into other queries. Below is an example.

I paid for the verification today and after just a few hours my account shows my phone was verified and they didn't even call me! If this is what they call verifying people I will never again let the verified logo be a factor in the decision of joining a program.
